Consent to the Processing of Personal Data

To register you in our loyalty program and enable you to fully enjoy all of our marketing services and membership benefits without interruption or limitation, we require your consent to process your personal data for this purpose. The full text of the consent is provided below.

By confirming this consent, you also acknowledge that you have duly familiarized yourself with the general rules of the loyalty program. The General Terms and Conditions of the Loyalty Program, as well as detailed information on the protection and processing of personal data by the operator, are available at www.citycasino.sk and at all of the operator’s establishments. Pursuant to Act No. 18/2018 Coll. on the Protection of Personal Data, as amended, and Regulation (EU) 2016/679 of the European Parliament and of the Council (hereinafter collectively referred to as the “Act”), I hereby consent to the processing, storage, and collection of my personal data, including: title, first name and surname, date of birth, personal identification number, permanent or other residential address, correspondence address (if applicable), email address, telephone number, ID card number or other identification document number (passport, driver’s license, or residence permit), information regarding registration in the loyalty program or participation in gambling activities at a gaming hall/casino, and information regarding winnings. These personal data may be processed by the operator: AND – JUR 4 s. r. o., Cintorínska 2, 942 01 Šurany, Company ID (IČO): 55193706 (hereinafter referred to as the “Operator”), as well as by entities contractually authorized by the Operator to process personal data, for the following purposes: commercial and marketing activities related to the provision of services within the loyalty program and other loyalty programs, in particular for maintaining your loyalty account, granting membership benefits, sending information about current promotions and competitions and ensuring your participation in them, offering our services, improving the quality and scope of our communication with you, and marketing analysis and profiling; the unique identification of clients (players), which the Operator is legally required to perform pursuant to Act No. 297/2008 Coll. on Protection Against the Legalization of Proceeds from Criminal Activity and Protection Against Terrorist Financing, as amended, as well as for the purpose of simplifying the provision of the Operator’s services. Your personal data are processed on the legal basis of your consent. If you do not grant this consent, such processing will not take place. I acknowledge that I have been informed and agree that the Operator processes and stores my personal data where such processing and storage are necessary for archiving purposes in accordance with Regulation (EU) 2016/679 (GDPR), as well as for my identification, which the Operator is legally obliged to perform pursuant to Act No. 297/2008 Coll., as amended, and in connection with the client’s consent for the provision of the Operator’s services. The Operator will not transfer personal data to third countries.

The consent you have provided above is voluntary and may be withdrawn at any time. Withdrawal of consent shall not affect the lawfulness of the processing carried out prior to such withdrawal. This consent to the processing of personal data is granted for a limited period, namely for as long as necessary to fulfil the agreed purposes for which the personal data are processed. The primary period of validity of this consent is the duration of the contractual relationship with the Operator and, following its termination, for a period of ten (10) years. I voluntarily, freely, seriously, and unequivocally grant my consent to the processing of the above-mentioned personal data by completing this application form. I declare that I have full legal capacity and that all personal data provided by me are accurate and truthful. Furthermore, I declare that I am aware of my rights under Sections 19 and 30 of the Act and that, prior to my confirmation of this consent, the Operator duly fulfilled its information obligation pursuant to Section 15 of the Act and properly informed me in accordance with Section 21 of the Act. I have also been duly informed of my right to access my personal data, the right to request confirmation as to whether or not my personal data are being processed, the right to rectification of personal data, the right to erasure of personal data, the right to restriction of processing, the right to object to the processing of personal data, the right to data portability, the right to request a list of my personal data that are being processed, the right to supplement or block personal data, the right to file a motion to initiate proceedings pursuant to Section 100 of the Act, as well as the right to withdraw my consent to the processing of personal data at any time and to request the deletion of my personal data in accordance with the Act.
